COUNSELING FORMS
Please download and read the five PBC Counseling documents. Complete the three forms we need back from you, including the Basic Information Questionnaire, the Personal Data Inventory, and the Consent to Counsel forms. Then return them to PBC. They need to go to Craig Swain, the elder coordinating our counseling ministry.
Once you have downloaded and completed the forms, sign them and either send in the paper copy or scan them and email them to Craig at cswain@providence-bc.com. To send in paper copies, you can mail them to the church or drop off an envelope at:

You can also give them to any elder at the church and they will get them to Craig. Once received, Craig will review the documents, speak with you and our counselors, and schedule two appropriate counselors to work with you.
